My Aged Care

Accessing My Aged Care is the first step in receiving Over 65’s services with CRANES.

You can call them on 1800 200 422 and speak with one of the staff on how to get you onto the road of receiving the help that you need.

The My Aged Care Team will determine your eligibility to receive services and they will then refer you to a service provider such as CRANES that will be able to support your identified need.

Examples of services: Socialisation, Shopping Assistance, Personal Care, Domestic Assistance, Lawn and Garden Maintenance, Allied Health Services, Home Modifications, the list is endless.  The support is there to assist you to remain living in your home for a long as possible.

Commonwealth Home Support Programme 
(CHSP)

CRANES is funded to provide services to people across the Clarence Valley to enable them to stay in their homes for longer, to be able to provide social and recreational support, provide carers with a break from their caring role and to generally provide support when needed.  If this is what you are looking for CRANES can assist you to do this.

All of our services have a strong focus on reablement and wellness to assist the person to regain their functional capacity, improve their independence and quality of life.

Care Management

At CRANES you will have a dedicated customer care coordinator who will support you to create a unique plan that best suits your support requirements and lifestyle.  Your care coordinator will check in on you regularly to ensure that services and supports are in line with your home care budget and meeting your changing needs.

   

Package Management

As the manager of your Home Care Package, CRANES will ensure that your package is utilised and monitored in line with all standards, legal and legislative requirements.

CRANES is an approved provider to provide Home Care Packages.

Domestic and Yard Services (Sparkle Flock)

CRANES can assist you to care for your home just the way you like it.  We want to assist people to be able to live independently in their homes by providing assistance with household duties to ensure that your home stays safe and well maintained.  

Whether you need the floors cleaned, lawns mowed, or beds changed, our experienced staff will be there to assist you.  We understand that everyone likes tasks performed differently and our coordinators will work with you to develop a plan for services on how you would like things done.

Your Home Care Package can fund this or you can pay for this service privately.  A no obligation free quote can be provided to you should you choose to pay for this not from your plan.

Flexible Respite 1:1 Support

This support can be provided in the home or out and about in the community.  The support is tailored to meet each person goals and aspirations. 

Some of the activities may include fishing, arts and crafts, shopping, morning tea or lunch out at your favourite café.  The list is endless…

Hubs & Clubs - Sunshine Clubs

There are currently 7 active groups across the Clarence Valley (Grafton @ 4 groups, Yamba, Broomshead and Glenreagh).  These groups meet either weekly or fortnightly.  The group meets within the centre of each of the above communities. 

Social connections, activities, community access and special guests are a part of the Sunshine Club’s usual program plan.  Transport is provided to and from the venue from the person’s home.  Morning tea and lunch are also provided.

Social Suport

Social Support enables people to access their community for 1:1 support or small groups with people who have similar interests.  There are shopping groups, active groups out and about in the community or just support to enable new connections to be made.  These connections can be social friendships for support or connections back to their community. 

Phone support is also provided for those who choose to remain at home and not go out and about.  We also provide phone calls to support and provide social connection to the person remaining in their home.

Brokerage Services

CRANES can provide you with the service of your choice at a fee for service rate. 

This can be for Support in the community 1:1 or in a group setting, support around and in your home using our Sparkle Flock Service. 

The choice is endless, and the support is tailored to suit your needs.

Allied Health

Allied Health Services can be paid for such as occupational therapist, physiotherapist, speech therapist etc.

 

These services are time-limited and could be referred for home modification or equipment assessment or to focuses on reablement and wellness to assist the person to regain their functional capacity, improve their independence and quality of life.

Transport

Transport can be arranged to attend a medical appointment with or without support or to get the person to their chosen activity in the community such as attending a Sunshine Club on a weekly basis. 

 

Support is provided door to door.
